This file contains information on the providers referenced in the FHIR bundle. This could contain individual providers or facilities.
Columns
provider_id
string
The practitioner ID from the FHIR bundle field practitioner.id.
local_identifier
string
The first business identifier from the provider’s identifier field where the use type is usual.
npi
string
The National Provider Identifier of the provider, if available.
tin
string
The taxpayer identification number of the provider, if available.
lastname
string
The last name of the provider, if the provider is an individual. If this row represents a facility, this field will contain the facility name.
firstname
string
The first name of the provider, if this row represents an individual. For facility providers, this will be blank.
middlename
string
The middle name of the provider, if this row represents an individual. For facility providers, this will be blank.
phone
string
The phone number of the provider.
state
string
The state in which the provider is located.
postalcode
string
The ZIP code in which a provider is located.
practitionerrole_specialty_code, practitionerrole_specialty_display, and practitionerrole_specialty_code_system
string
The provider specialty, if available.
metadata_id
string
Reflects the origin of the data. Uniquely identifies a row in metadata.csv.
